I/We hereby give permission for my child/children to participate in St. John's children and youth activities. I understand that reasonable precautions will be exercised by the staff and volunteer leaders and that at least one parent or guardian will normally be present on Sunday mornings. Neither the volunteers nor St. John's Church will be responsible for theft or damage to personal property, nor for boldly injury. I understand that under ordinary circumstances accidents can occur and that injuries can result. Unless otherwise notified, I certify that my child is of normal health and is physically able to participate in church activities. Any allergies, medications or special needs have been noted on this form. In the event of an accident or an illness while at church and attempts to contact a parent are unsuccessful, I give permission for my child to receive emergency medical treatment as deemed necessary by emergency medical personnel. This permission remains in effect from September 1, 2023 - August 31, 2024 unless it is terminated by me in writing.

I/We authorize St. John’s Episcopal Church in McLean, Virginia to use and/or display photography of my child in publications, multimedia productions, displays, advertisements or internet publication related to the promotion of St. John’s Episcopal Church or in the promotion of any activities, events, or programs supported by St. John’s Episcopal Church. The child’s name will not be disclosed should images be utilized. In granting this consent, I/we agree to release and forever discharge St. John’s Episcopal Church and its staff members, agents, and employees from any and all claims and demands arising out of or in connection with the use of said photographs / images, including but not limited to, any claims for invasion of privacy or defamation. This authorization is valid from September 1, 2023 - August 31, 2024 unless it is terminated by me in writing.
